Jubilee of Youth Group/Family Registration

  Use this form to register your ADW parish group/family to attend the 2025 Jubilee of Youth as a part of the ADW Catholic Youth Organization pilgrimage. Group leaders/families must submit their deposit along with this form to secure your spot. Registration forms are not considered complete until payment is collected.

  This pilgrimage has a limited number of spaces available. Registrations are accepted on a first come, first serve basis. After submitting this form you will be contacted by the CYO Office to confirm/finalize your registration. All confirmed registration fees are non-refundable. If spaces are no longer available your deposit will be refunded and you will be placed on a waitlist.
  
  Groups should try to have a minimum of 2 adult chaperones, for small groups of minors. Additional adult chaperones should be added to maintain a 5 minor youth: 1 adult chaperone ratio. Adult chaperones must be 21 years of age or older and meet the requirements of The Roman Catholic Archdiocese of Washington Child Protection policy including, but not limited to being Virtus trained, fingerprinted and under an active status.
