on behalf of myself, my heirs, assigns, Executors, and personal representatives, to hold harmless and defend The Roman Catholic Archdiocese of Washington; Wilton Gregory, Roman Catholic Archbishop of Washington; a Corporation Sole: the Catholic Youth Organization of Washington, DC and Metropolitan Area, Inc; the Office of Youth Ministry; Msgr. O'Dwyer Retreat House their employees; volunteers, coordinators and chaperones from any and all liability claims, loss or damage arising from or in connection with my participation in Leaders Inspired to Evangelize (L.I.T.E.) held at Msgr. O'Dwyer Retreat House on Sunday, June 25, 2023 through Thursday, June 29, 2023.

I hereby grant permission to the Catholic Youth Organization of Washington, DC and Metropolitan Area, Inc, the Office of Youth Ministry, and The Roman Catholic Archdiocese of Washington to use any photographs and quotations taken of me during Leaders Inspired to Evangelize (L.I.T.E.) to assist in community awareness, educational efforts, related public relations purposed that my include brochures, posters, website and print media. I agree to abide by all rules and regulations as outlined in the Code of Behavior.

The following guidelines will ensure a fun and safe experience for all of us:

1. Participants will be given credentials, which must be worn at all times.
2. Participants are expected to attend all events on time and participate fully.
3. Participants are expected to remain with their parish group at all times.
4. In keeping with the laws of the U.S.A. minors will not be permitted to consume alcohol. The possession of illegal drugs is clearly prohibited and is cause for dismissal.
5. The Parish/Organization & adult chaperone are responsible for the overall actions of their participants.
6. In accordance with CYO policy, adults will refrain from consuming alcohol during Leaders Inspired to Evangelize (L.I.T.E.). Adults may not consume alcohol during any youth oriented event and while serving as chaperones to minors.
7. All participants’ adults and youth will be held to the highest Christian standards of morality.
